- A scan of all the pages of the applicant’s passport in one pdf file.
- The applicant’s birth certificate, issued this month.
- A questionnaire filled out.
- Please indicate weather you have an orthodox Rabbi that knows your family that can write a letter stating that you are Sephardi. if not, we will assist in this.
Children under 18 cannot file a request. Only children born after the citizenship is granted automatically receive citizenship.
